If you are applying for services for horses, including assistance for grain, hay, basic vet care, or other equine needs, please fill out the contact information and add a summary of the horse’s situation and the needs of the horse, and submit. A formal application will be emailed to you, and if you are eligible for services, assistance will be provided to the horse or horses, as funding permits.
